Requirements

Handwritten Personal Data Sheet (PDS) (CSC Form 212, revised 2017 duly notarized). Print entries using BLACK BALLPEN. Do not use sign pen. Use blue pen in affixing signature. Original/Certified True Copy of College Transcript of Records and College Diploma (by school); f. Original/Certified True Copy of Certificate of Waiver issued by Chief, BFP (for applicants with Age and/or Height deficiency/ies), if applicable; g. PSA/NSO Certificate of Live Birth (If not readable, provide Original/Authenticated Copy of Certification of Birth from the Local Civil Registrar); h. PSA/NSO Marriage Certificate (if applicable) (If not readable, provide Original/Authenticated Copy of Certification of Marriage from the Local Civil Registrar); i. Photocopy of the Second Level Eligibility/Report of Rating from Civil Service Commission (CSC)/Eligibility administered by the Professional Regulations Commission (PRC); (For applicants with PRC Eligibility, submit photocopy of unexpired PRC ID and Certificate of Board Rating or Board Passer); j. Photocopy of Pag-ibig Member Data Form (MDF) and Photocopy of Philhealth Membership Data Record (MDR); l. Photocopy of valid Professional Driver's License ID and/or NC II/III in driving (if available); and m. Original/Certified True Copy of valid clearances from Barangay, Philippine National Police, NBI, RTC, and MTC; Note: Applicants with previous employment are required to submit additional clearances obtained from their previous employer whichever is applicable: Company/Agency clearance, IAS clearance, Certificate of Non-Pending Case, Certification of Honorable Dismissal, and the like.
